I was last time in Altering cave like 10 years ago, honestly it's a very old location from like 2013 and 2014 when not all pokemons were available in the wild. There was a small bunch of pokes that were available only through this location but there is no point of going there anymore. Indeed, you are correct- in order to access Islands 4-7, you need to beat E4 and Champion. Afterwards, talk to Celio- he will send you on a quest to obtain Sapphire, which is located in Mt. Ember cave, where 2 rockets are talking with each other. This is all done in Kanto. You can use this guide, it's one of the best guides the forums have to offer and it will take you by hand, allowing you to collect hidden items and complete the storyline post-e4.

Outrage is 90 BP in PokeMMO, because it's buff was idiotic and made dragons powerful to a degree that they had to introduce the fairy type in Gen6. Having 120 base power Outrage in MMO would be horrible for all of the metagames, regardless if it's singles or doubles. While I do not agree with most decisions made by the dev team, like buffing Gallade and fucking around with it's ability or nerfing Wabba, this is a good decision from their side. As long as we will not have Gen6+ mechanics and pokes, we should never have 120bp Outrage.

You get one or two Spiritombs from storyline using Odd Keystones. Start switching channels around the most popular Feebas spots in Hoenn (southern part of the route where it spawns). If it will not allow you to switch a channel, BINGO. Ice gym is actually one of the easiest ones, at this point you should have access to multiple fire/ fighting types in the region. In Celestial Tower you can catch a Litwick which you can evo into Chandelure as soon as it reaches lvl 41, making things much easier for you. For now, I'd suggest buying a cheap pokemon which can learn Sweet Scent so you can quickly level a pokemon, while giving it the EVs it would appreciate most (Chandelure likes spatk and speed).
